As crucial interface organs gut and skin have much in common. Therefore it is unsurprising that several gut pathologies have skin co‐morbidities. Nevertheless, the reason for this remains ill explored, and neither mainstream gastroenterology nor dermatology research have systematically investigated the ‘gut‐skin axis'. Here, in reviewing the field, we propose several mechanistic levels on which gut and skin may interact under physiological and pathological circumstances. We focus on the gut microbiota, with its huge metabolic capacity, and the role of dietary (...) components as potential principle actors along the gut‐skin axis. We suggest that metabolites from either the diet or the microbiota are skin accessible. After defining open key questions around the nature of these metabolites, how they are sensed, and which cutaneous changes they can induce, we propose that understanding of these pathways will lead to novel therapeutic strategies based on targeting one organ to improve the health of the other. Background:The process of dying in intensive care units is complex as the technological environment shapes clinical decisions. Decisions at the end of life require the involvement of patient, families and healthcare professionals. The degree of involvement can vary depending on the professional and social culture of the unit. Nurses have an important role to play in caring for dying patients and their families; however, their knowledge is not always sought.Objectives:This study explored nurses’ care practices at the end of life, with (...) the objective of describing and identifying end of life care practices that nurses contribute to, with an emphasis on culture, religious experiences and professional identity.Research Design and context:Grounded theory was used. In all, 10 nurses from intensive care unit in two large hospitals in Bahrain were participated.Ethical Considerations:Approval to carry out the research was given by the Research Ethics Committee of the host institution, and the two hospitals.Findings:A core category, Death Avoidance Talk, was emerged. This was supported by two major categories: (1) order-oriented care and (2) signalling death and care shifting.Discussion:Death talk was avoided by the nurses, doctors and family members. When a decision was made by the medical team that a patient was not to be resuscitated, the nurses took this as a sign that death was imminent. This led to a process of signalling death to family and of shifting care to family members.Conclusion:Despite the avoidance of death talk and nurses’ lack of professional autonomy, they created awareness that death was imminent to family members and ensured that end of life care was given in a culturally sensitive manner and aligned to Islamic values. Dandruff is a common scalp condition, which frequently causes psychological distress in those affected. Dandruff is considered to be caused by an interplay of several factors. However, the pathogenesis of dandruff remains under‐investigated, especially with respect to the contribution of the hair follicle. As the hair follicle exhibits unique immune‐modulatory properties, including the creation of an immunoinhibitory, immune‐privileged milieu, we propose a novel hypothesis taking into account the role of the hair follicle. We hypothesize that the changes and imbalance of (...) yeast and bacterial species, along with increasing proinflammatory sebum by‐products, leads to the activation of immune response and inflammation. Hair follicle keratinocytes may then detect these changes in scalp microbiota resulting in the recruitment of leukocytes to the inflammation site. These changes in the scalp skin immune‐microenvironment may impact hair follicle immune privilege status, which opens new avenues into exploring the role of the hair follicle in dandruff pathogenesis. Before COVID-19, dementia singing groups and choirs flourished, providing activity, cognitive stimulation, and social support for thousands of people with dementia in the UK. Interactive music provides one of the most effective psychosocial interventions for people with dementia; it can allay agitation and promote wellbeing. Since COVID-19 has halted the delivery of in-person musical activities, it is important for the welfare of people with dementia and their carers to investigate what alternatives to live music making exist, how these alternatives are (...) delivered and how their accessibility can be expanded. This community case study examines recent practice in online music-making in response to COVID-19 restrictions for people with dementia and their supporters, focusing on a UK context. It documents current opportunities for digital music making, and assesses the barriers and facilitators to their delivery and accessibility. Online searches of video streaming sites and social media documented what music activities were available. Expert practitioners and providers collaborated on this study and supplied input about the sessions they had been delivering, the technological challenges and solutions they had found, and the responses of the participants. Recommendations for best practice were developed and refined in consultation with these collaborators. Over 50 examples of online music activities were identified. In addition to the challenges of digital inclusion and accessibility for some older people, delivering live music online has unique challenges due to audio latency and sound quality. It is necessary to adapt the session to the technology's limitations rather than expect to overcome these challenges. The recommendations highlight the importance of accessibility, digital safety and wellbeing of participants. They also suggest ways to optimize the quality of their musical experience. The pandemic has prompted innovative approaches to deliver activities and interventions in a digital format, and people with dementia and their carers have adapted rapidly. While online music is meeting a clear current need for social connection and cognitive stimulation, it also offers some advantages which remain relevant after COVID-19 restrictions are relaxed. The recommendations of this study are intended to be useful to musicians, dementia care practitioners, and researchers during the pandemic and beyond. (shrink)

This article explores the role of nature in two strands of contemporary materialist philosophy: new materialism, and transcendental materialism. Through an analysis of these strands of materialism via the work of Jane Bennett, William E. Connolly, Catherine Malabou, and Adrian Johnston, the article attempts to delineate these perspectives into the opposed camps of monist and dialectical materialisms. The implications of these differing materialist ontologies are then discussed in terms of the theorization of nature as either a vital material force (...) or as a weakness characterizing matter itself. After outlining the importance of nature for these two opposed theories, the lack of serious engagement with the work of F.W.J. Schelling by either group is discussed as a potential middle ground between the Spinozism of the new materialism and the Hegelianism of transcendental materialism, due to its large-scale political analysis, less pessimistic perspective towards the possibility of a contemporary humanism, and embrace of recent advances in the empirical sciences. This article sets the agenda for a new materialist humanism through a critique and analysis of theories of materialist subjectivity in recent French philosophy. I begin with a critique of the lack of a properly internal account of the human subject in the work of Alain Badiou, arguing that his disavowal of any sort of humanism and a dismissal of the natural sciences leaves him without a way to conceptualize the internal activity of the human subject. I then consider the (...) neurological account of human subjectivity offered in Catherine Malabou's recent materialist articulation of human consciousness and consider how this internal theory of human subjectivity can supplement the wholly external account offered by Badiou. I conclude the article with a consideration of Quentin Meillassoux's speculative materialism to show the manner in which at the ontological level he offers a materialist account of human thought as contingently emerging from wholly material processes. Building upon this, I point towards the possibility of a non-hierarchical form of materialist humanism that is wholly immanent in its affirmation of the unique capacities of human thought and activity without claiming any special metaphysical significance for human beings. The subject of this paper is the integrity of nature over time—‘diachronic integrity’. The argument of the parer is that any serious attempt to address conservation problems—the kinds of problems faced by environmental managers the world over, needs to operate with an eye to some principle of diachronic integrity. Whilst acknowledging that applying the principle is largely a matter of experience and judgement, we argue that it applies equally both to human and to natural history.

Peter Lombard lamented the abandonment of Augustine’s position affirming the materiality of demons and the demonic body, since by his time (some 700 years after Augustine), under the influence of the Pseudo-Dionysius, it was generally agreed within the Christian tradition that demons (and angels) are intelligible, disembodied substances. The principles that the cosmos is spatially and materially divided and stratified and that demons share ontologically in the nature of the part that they inhabit allowed figures such as Apuleius, Porphyry, and (...) Augustine to account for various aspects of demonic nature and activity that seemed to follow from reason, revelation, and practical experience. The material body of the demon and its relation to the pneuma, or vehicle of the soul, is often invoked to explain demonic agency and the extent of demonic knowledge and to distinguish between evil and good demons (or angels). The following paper weighs in on and clarifies Augustine’s demonic ontology. Although scholars generally recognise Augustine’s position maintaining that demons possess aerial bodies, nevertheless, the ambiguity surrounding Augustine’s opinion, attested to even by Peter Lombard, is overlooked, and Augustine’s conviction is often taken for granted and supported by citing the prominent City of God, the very work in which Augustine’s own views are suspect. This paper provides support to the assessment of Augustine’s doctrine of the demonic body by examining his cosmology, particularly his spatial and elemental division of the cosmos, and exhibits his adherence to the spatio-material principle according to which a creature’s bodily constitution is cognate with the elemental composition of the physical stratum of the cosmos in which they dwell. The paper concludes by indicating how Augustine’s explanations of demonic agency, far from being ambiguous on the nature of the demonic body, are in fact premised on their aerial corporeality, thereby demonstrating more definitively his conviction that demons possess material bodies. In 1690 a Latin translation of a philosophical treatise, originally written in English by Anne Conway , was published anonymously. The English manuscript did not survive, but in 1692 the Latin version of Conway's text was translated (...) into English as The Principles of the Most Ancient and Modern Philosophy. Conway was widely known by seventeenth-century philosophers and religious writers, including the Cambridge Platonist, Henry More; Descartes's correspondent, Princess Elisabeth of Bohemia; Robert Boyle; physician and vitalist philosopher, Francis Mercury van Helmont; as well as numerous Quaker leaders. And her Principles were read with critical approbation by such figures as Leibniz. Further, Conway's text is probably the most anthologized of the writings of seventeenth-century women philosophers. It is curious, then, to discover a dearth of critical literature on the Principles. When Peter Loptson produced the first modern edition of the Principles , he attempted to interest contemporary philosophers in Conway's essentialist metaphysics. In a recent article, ?Marxism and Radical Democracy?,1 Femia argues that Marxism is incompatible with radical democracy. In so doing he specifically reiterates2 a now common claim that the notion of scientific socialism defended by Marx and Engels and prevalent in the Second International is anti?democratic. This claim has not only been made by critics of Marxism.3 It has been a major criticism of classical Marxism within the Western Marxist tradition, in particular? in the work of the Frankfurt School.4 It (...) is one of the main reasons why the classical Marxism of Engels and the Second International has been rejected as positivist and vulgar: no modern sophisticated Marxist admits to either positivism or vulgarity. In this paper I examine and reject Femia's arguments for the claim that the notion of scientific socialism is undemocratic. I argue that the orthodox view of Marxism as a scientific theory is compatible with democracy, and indeed encourages a democratic understanding of socialism. ABSTRACT Is a scientific attitude to the natural world an obstacle to an appreciation of its value? This paper argues that it is not. Following Aristotle and Marx, it maintains that, properly pursued, science has value because it enables us to contemplate that which is wonderful and beautiful. However, the paper concedes that, as actually practised, science can foster a vice described by Augustine as ‘the lust of the eyes’: knowledge is sought not to open us to the world, but (...) merely to satisfy the itch of curiosity. If scientific knowledge is thus pursued, no limits to the means to it nor to its objects are recognised. Those who thus seek knowledge fail to understand its value. (shrink)

ABSTRACT It is widely supposed by both its proponents and critics that communitarianism is committed to the defence of lies of nationhood: the nation forms a surviving communal attachment in a world in which the individual is otherwise denuded of ties of community. I argue in this paper that this assumption is mistaken. It depends on a romantic image of the nation which was constructed in the eighteenth and nineteenth centuries. That image hides the recent historical origins of the nation (...) and its foundation in the suppression of older allegiances or their later privatisation; it continues to disguise the illusory nature of the national community. The nation is not a surviving pre‐modern community to which individuals are attached as a bulwark against the impersonality of public life in modern society. Rather the nation is one of the main vehicles for the construction of the modern unencumbered self. Communitarians need not be committed to the defence of the specifically modem forms of public irrationality and immorality that nationalism embodies. (shrink)

ABSTRACT In a recent paper Alan Carter argues that the claim that workers’co‐operatives merely replace exploitation by employers with ‘self‐exploitation’is nonsense: the term ‘self‐exploitation’is self‐contradictory. He maintains that the only form of exploitation to which a workers’co‐operative may be said to be subject is ‘market‐exploitation’by dominant economic actors who are external to the co‐operative. I argue that these conclusions are mistaken. While the concept of ‘market‐exploitation’is not without value, it is difficult to operationalise. While the concept of ‘self‐exploitation’is, understood literally, (...) incoherent, the claims defended by some of the users of this term can be made conceptually respectable. (shrink)

Inchoate crime consists of acts that are regarded as crimes despite the fact that they are only partial or incomplete in some respect. This includes acts that do not succeed in physically harming the victim or are only indirectly related to such a result. Examples include attempts (as in attempted murder that does not eventuate in the killing of anyone), conspiracy (in which case the crime has only been planned, not yet acted out) and incitement (where the inciter does not (...) himself commit the crime he is urging others to undertake). The present paper attempts to analyze these inchoate crimes from a libertarian perspective, based on the non-aggression principle. (shrink)

This thesis examines some mind-body problems traditionally ascribed to Descartes' philosophy. One such problem focuses on inconsistencies in Descartes' general causal claims. Another problem, first put forward by Simon Foucher, concerns Descartes' purported espousal of the following inconsistent triad: mind-body causal interaction, mind-body distinctness, and "the causal likeness principle." The final problem is one regarding free will and determinism. ;In the first Chapter I examine the content and number of Descartes' causal principles. An analysis of the main concepts used in (...) these principles, for example, "perfection," "eminent containment," and "formal containment," is provided. This examination includes a discussion of some medieval antecedents of these concepts. I argue that there is no blatant inconsistency generated by Descartes' causal claims. ;In the second chapter I utilize the analysis of "eminent containment" to show that Descartes was not committed to a "causal likeness principle." Since this principle is crucial for generating the "Foucher mind-body problem," I thus demonstrate that this problem is not one that applies to Descartes' philosophy. ;In this chapter I also examine Descartes' views about the details of psycho-physical change. While there is no blatant inconsistency among these views, I show that Descartes is committed to the inexplicability and indeterminism of the acts of the will. This presents a problem given Descartes' acceptance of psycho-physical interaction and of Laplace-like determinism. ;In the final chapter I argue that Descartes could have avoided this problem by denying Laplacean determinism, but since he lacks a clear position on the matter, it is not the move he himself makes. I also show how Descartes vacillates between a compatibilist and an incompatibilist view of free will. (shrink)

The necessity for a government “lender of last resort” has been advanced as a justification for central banking. In this paper, I compare lending practices under central banking with those that would be likely to exist under a system of fractional-reserve free banking (FRFB). To do this I examine the underlying nature of banks as warehousing and credit-granting institutions and consider how redemption runs can arise as a consequence of fractional reserves in this system. Following the work of Thornton and (...) Bagehot, I describe principles of prudent lending that can be used to stem a redemption run when it arises. I examine the market incentives that apply under FRFB and how these incentives are perverted under a system of central banking. I find that a government central bank is not well-placed to lend according to prudent standards, and in fact is likely to use its power to pursue political goals that are at odds with prudent lending. I examine the US financial crisis and the actions of the Federal Reserve System to illustrate this view. (shrink)

People more frequently select norm-violating factors, relative to norm- conforming ones, as the cause of some outcome. Until recently, this abnormal-selection effect has been studied using retrospective vignette-based paradigms. We use a novel set of video stimuli to investigate this effect for prospective causal judgments—i.e., judgments about the cause of some future outcome. Four experiments show that people more frequently select norm- violating factors, relative to norm-conforming ones, as the cause of some future outcome. We show that the abnormal-selection effects (...) are not primarily explained by the perception of agency (Experiment 4). We discuss these results in relation to recent efforts to model causal judgment. (shrink)

Why has autonomy been a leading idea in philosophical writing on bioethics, and why has trust been marginal? In this important book, Onora O'Neill suggests that the conceptions of individual autonomy so widely relied on in bioethics are philosophically and ethically inadequate, and that they undermine rather than support relations of trust. She shows how Kant's non-individualistic view of autonomy provides a stronger basis for an approach to medicine, science and biotechnology, and does not marginalize untrustworthiness, while also explaining (...) why trustworthy individuals and institutions are often undeservingly mistrusted. Her arguments are illustrated with issues raised by practices such as the use of genetic information by the police or insurers, research using human tissues, uses of new reproductive technologies, and media practices for reporting on medicine, science and technology. ObjectivePatient involvement is used to describe the inclusion of patients as active participants in healthcare decision-making and research. This study aimed to investigate incoming year 1 medical (MBChB) students’ attitudes and opinions regarding patient involvement in this context.MethodsWe established a staff–student partnership to formulate the design of an online research survey, which included Likert scale questions and three short vignette scenarios designed to probe student attitudes towards patient involvement linked to existing legal precedent. Incoming year 1 medical students (n=333) were (...) invited to participate in the survey before formal teaching commenced.ResultsSurvey data (49 participants) indicate that students were broadly familiar with, and supportive of, patient involvement in medical treatment. There was least support for patient involvement in conducting (23.9%), contributing to (37.0%) or communicating research (32.6%), whereas there was unanimous support for patients choosing treatment from a selection of options (100%).ConclusionIncoming members of the medical profession demonstrate awareness of the need to actively involve patients in healthcare decision-making but are unfamiliar with the utility and value of such involvement in research. Further empirical studies are required to examine attitudes to patient involvement in healthcare. (shrink)

This article examines methodological and ethical issues of ethnographic research through the lens of Emmanuel Levinas's philosophy. Levinas is relevant to a critical analysis of ethnographic methods because his philosophy turns on the problematic relationship between self and other, among other important problems that define and guide contemporary anthropological research, including questions of responsibility, justice, and solidarity. This article utilizes Levinas's philosophy to outline a phenomenology of the “doing” of fieldwork, emphasizing the contingency of face-to-face encounters over controlled research design. (...) This account provides a basis for going beyond the polarized opposition between objective and subjective ethnographic approaches. Levinas allows for an ethically informed ethnography premised upon an acknowledgement of risk and uncertainity over researcher control or reflexivity. Providing a handful of concrete examples, the article argues that critical self-reflection about the fundamental face-to-face dimension of fieldwork is central to ethnography's ethical possibilities.
